Community Transport Calderdale is working in partnership with Age UK Calderdale & Kirklees to provide the ‘Home from Hospital’ service.

This is a free transport and support service for elderly and vulnerable patients in Calderdale and parts of Kirklees. We help with transport home after a hospital stay from places such as Calderdale Royal Hospital, Huddersfield Royal Infirmary and smaller units such as Moorland Grange Immediate Care Unit.

We provide a safe, supported, wheelchair- accessible journey home from hospitals in the local area. Patients can be met at home by Age UK personnel, who can offer immediate help and arrange any support if needed.
